Supported Living and Home Care Packages
Everybody needs a little help at one time or another. Our Supported Living Program enables you to choose from a variety of home care support services to help you remain living at home for as long as possible. These services may be provided under a private fee-for-service agreement, or a Government subsidised Home Care Package, or a combination of both.
We are an Approved Provider of Home Care Packages for residents who need additional support living at home and are providing these additional Home Care services at all sites.
Home Care Packages and Consumer Directed Care (CDC)
Consumer Directed Care means that you have more choice and more say in what services you receive in your Home Care Package, who provides your services, how they are delivered and how your budget is spent. Your Approved Provider and Case Manager will work with you to develop and review your care plan to ensure your Home Care Package services continually meet your individual needs and goals.
Eligibility for a Home Care Package
You may be eligible for a Home Care Package if you are
- An older person who needs coordinated services to help you stay in your home.
- A younger person with a disability, dementia or other special care needs that are not met through other specialist services.
